  1. Prepayment may be required until this application is processed and approved.
  2. Canceled orders are subject to fees incurred and a cancellation fee.
  3. The 1% discount does not apply to credit card payments, freight, or postage.
  4. A $40 surcharge will be billed for all returned checks regardless of reason.
  5. A 1.5% monthly interest charge will be applied to any unpaid balance.
  6. Applicant is responsible for all legal and collection fees required to recover funds that are 60 days or more delinquent.
  7. Applicant authorizes Quick Tab II to verify the trade references provided and any additional financial references obtained separately as part of the application review process.
